Welcome to Internal Transitions LLC

Welcome!
I'm so glad you're here.
At Internal Transitions, LLC, I believe that meaningful collaboration is at the heart of compassionate care. Whether you're a fellow wellness practitioner, a hospice professional, a healthcare provider, or someone passionate about shifting the way we approach end-of-life care—I’m honored to connect with you.
I specialize in providing emotional, spiritual, and educational support to individuals and families navigating the final stages of life. I work alongside hospice teams, caregivers, and community organizations to offer a deeper layer of presence, peace, and personalized care.
While I bring my clinical background as a registered nurse, my role as a doula is non-medical—focused entirely on the emotional and human side of dying. Through sacred listening, practical guidance, and gentle companionship, I aim to create a space where people feel seen, supported, and empowered.
I also believe deeply in education as a tool for change. Through workshops and community conversations, I help dismantle fear around death and build understanding—so families and care teams alike can move forward with more confidence and clarity.
Together, we can foster a more compassionate and holistic approach to the end of life—one that honors dignity, comfort, and connection.
I’m so excited for the possibility of working together.
With warmth and gratitude,
Taressa Flores

Are you caring for an elderly parent and finding it hard to navigate this chapter of your life? Many people are finding themselves in the "sandwich generation" caring for children and their elderly parents while working full-time. I can help. If your loved one got a terminal diagnosis today, would you know how to care for them?
Medicare states that 98% of hands-on end-of-life care is done by family caregivers, even with hospice services on
board. There is help available.
I would love to discuss your "aging well" plan. Life expectancy has almost doubled in the last 100 years. The elderly population is one of the fastest-growing demographics. With advanced age comes several potential challenges: physically, cognitively, and financially. We don't know how long we will live and what the circumstances will be when we do. It is best practice and recommended to have a Living Well Aging Plans A, B, and C in place to cover all possibilities. Empower yourself by choosing what you want ahead of time.
"We had the pleasure of having Taressa as a doula during my grandmother's passing. She not only was extremely comforting for the whole family but for my grandmother as well. She honored her wishes & what she wanted to have done in the ending of her life. She also made sure that she had conversations with those who are the most important to her before she passed. Having Taressa there with us was a blessing in disguise. She came in like a beacon of light & gave us what we needed during that time. We will forever be grateful for her and her expertise." - Arie. O.
“A hundred years ago, end-of-life care was a skill that was handed down from generation to generation,” said O’Brien. “Our goal is to give everyone
the peace of mind that comes with knowing that when the time arrives, they will be better prepared to help their dying loved one face death without fear and anxiety, and will be able to spend that final sacred phase in presence, peace, and comfort.”
